What causes lower leg pain?

Lower leg pain generally falls into three broad groups. A careful assessment tells us which is at play — and what to do about it.

Musculoskeletal

Shin splints, calf strain, calf tightness and cramps, or compartment syndrome — pain from the muscles, tendons and joints.

Neurological

Nerve-related pain, numbness, tingling or weakness that can be referred into the lower leg.

Vascular

Circulation-related pain, such as an ache when walking that eases with rest — which we recognise and refer on when needed.

When lower leg pain needs prompt attention

Some lower leg pain has a circulation (vascular) cause. If you notice any of the signs below — or sudden pain, swelling, warmth or redness in one calf — please seek medical advice promptly, as these can indicate a circulation problem or a blood clot that needs urgent assessment.

  • !A painful ache in the legs when walking that eases after a few minutes' rest (intermittent claudication)
  • !Numbness or weakness in the legs
  • !Hair loss on the legs and feet
  • !Brittle, slow-growing toenails
  • !Non-healing ulcers (open sores) on the feet or legs
  • !Changes in skin colour — paler than usual, or a blue tinge
  • !Shiny skin, or shrinking of the leg muscles
